2012 Trust Rewards program—Health Assessment completion is the only required activity.

We've made the following changes, including an earlier payout of the reward:
· Health Assessment (HA) completion is the only requirement to receive a $25 prepaid card.
· Eligible members will receive their $25 prepaid card about eight weeks after HA completion.
· Completion of the satisfaction survey in 2012 is voluntary—it is no longer required.
· New Web portal simplifies the registration process.

Trust in Your Health is available to subscribers and eligible spouses/domestic partners who have WEA Trust as their primary health insurer. Complete the HA between January 1 and December 31, 2012, and receive a $25 prepaid card and chances to win other prizes throughout the year.

2011 Trust Rewards program—still time to earn a $50 prepaid card! The $50 prepaid cards for the 2011 Trust in Your Health/Trust Rewards Program will be mailed to members' homes by early February 2012. To earn the reward, members need to complete the online health assessment, the online satisfaction survey, and earn 15 Reward points by midnight of December 31, 2011. In addition, members need to have the Trust health plan as their primary insurance on January 1, 2012. Members often overlook the satisfaction survey requirement.
